Pre-participation
Exam and Insurance Forms:
On behalf of the athletic training staff, we would
like to welcome you to Kalamazoo College. We trust your
experience here will be enjoyable and will have a positive
impact on your future career goals. With that being
said there are a couple of things that we need from
you before you can begin your intercollegiate career
here at K.
VERY IMPORTANT
ATTENTION INCOMING FRESHMAN:
You will not be eligible this upcoming athletic season if the following is not completed and returned by AUGUST 1, 2009. |
Freshman and Transfer Student-Athletes:
You must have the following forms on file with the athletic trainer prior to being cleared to tryout, practice or compete in intercollegiate athletics at Kalamazoo College.
1. PPE - Pre-participation Physical Evaluation Form (PDF)
• Required of all incoming freshman/transfer student-athletes.
• Complete this form in its entirety and leave no question blank.
• Make sure that all the information is as accurate as possible, by going through this form with your parents, especially the medical history section.
• Page 3 MUST BE FILLED OUT BY A PHYSICIAN. Make an appointment with your family physician as soon as possible. This ensures that you are medically cleared to participate in intercollegiate athletics at K. Also bring the enclosed *Immunization History Form (PDF) to your appointment to have your healthcare provider complete and sign. Return this form to the Student Health Center.
• Failure to do any of these things will result in your ineligibility to participate in the 2009-2010 athletic season.

All students enrolling at K:
Separate from and in addition to the above forms all entering students must complete Entrance Medical Records, which includes the following information:
- Medical History Form
- Tuberculosis Self-Screening Form
- Immunization Form*
- Copy of both sides of your insurance card
You can complete the Medical History and Tuberculosis Self-Screening Forms on-line at https://reason.kzoo.edu/healthcenter/, download and print to complete them by hand, or contact the Student Health Center to request them mailed or faxed to you. Please contact the Student Health Center with questions at 269-337-7200 or healthsv@kzoo.edu.
*Return Entrance Medical Records to the Student Health Center by mail or fax (Medical History and Tuberculosis Self-Screening forms completed on-line and do not need to be mailed):

Incomplete Entrance Medical Records will result in an inability to participate in athletics, LandSea, move into residence halls, and start classes.
Returning Student-Athletes:
1. All you need to fill out and return is the Parent
Insurance Information Form .
2. You will have
a Medical Update done when you return to campus.
The Pre-Participation Physical Evaluation Form and Parent Insurance Form must be completed and returned no later than August 1, 2009 |
