Pre-participation
Exam and Insurance Forms:
On behalf of the athletic training staff, we would
like to welcome you to Kalamazoo College. We trust your
experience here will be enjoyable and will have a positive
impact on your future career goals. With that being
said there are a couple of things that we need from
you before you can begin your intercollegiate career
here at K.
Freshman and Transfer Student-Athletes:
You must have these
two forms on file with the athletic trainer prior to
being cleared to tryout, practice or compete in intercollegiate
athletics at Kalamazoo College.
- Pre-participation Physical Evaluation Form
• Required of all incoming freshman/transfer student-athletes.
• Complete this form in its entirety and leave no question blank.
• Make sure that all the information is as accurate as possible, by going through this form with your parents, especially the medical history section.
• Page 2 MUST BE FILLED OUT BY A PHYSICIAN. Make an apppointment with your family physician as soon as possible. This ensures that you are medically cleared to participate in intercollegiate athletics at K.
• Return BY AUGUST 1, 2007. to Scott Michel, Head Athletic Trainer, Kalamazoo College, 1200 Academy, Kalamazoo MI 49006 (FAX) 269.337.7401
• IN ADDITION, the Health Center Forms (online health history, TB and immunization forms) must be returned by August 1 to the Health Center or you will not be able to move in to your dorms for pre-season camp.
• Failure to do any of these things will result in your ineligibility to participate in the 2007-2008 athletic season.

Insurance Information Form
Returning Student-Athletes:
1. All you need to fill out and return is the Parent
Insurance Information Form.
2. You will have
a Medical Update done when you return to campus.
The Pre-Participation Physical Evaluation Form and Parent Insurance Form must be completed and returned no later than August 1, 2007 |
