A Health Questionnaire must be submitted before your first order will be filled. Regardless of where you live, the order form and questionnaire should be sent to MEDCO, PO Box 182050, Columbus OH 43272.

You can also register as a member at MedCo's website  When entering the contract number from your ID card, do not include any letters that may precede the number.  You might need to add a zero at the beginning. You must register before accessing forms or other information. Paper forms are also available in the Human Resources Office.

For new enrollees:  to show evidence of coverage before you receive your BCBSM membership card, you may print the appropriate coverage letter.
