Eligibility for most benefits requires at least half time employment.  Some benefits have eligibility requirements of three quarter or full time employment.  Please consult Human Resources staff for more information.

When comparing 2011 options, please note that the 90% Plan includes an automatic Health Reimbursement Arrangemen (HRA) that reduces the actual Preferred Benefit (in-network) deductible by 75%, and also, during this 2011 transition year only, will reduce the Alternate Benefit (out-of-network) deductible.  The maximum combined in- and out-of-network deductible reduction is $1,500 single and $3,000 double or family.  You will not need to file a claim; Priority Health will send payment directly to the provider.

Flexible Benefit Plan
Over-the-counter (OTC) drugs cannot be reimbursed from Flexible Spending Accounts unless a prescription is obtained for the drug. This does not apply to insulin, which continues to be an eligible medical expense, even if purchased without a prescription.
