(Please note that Host Institutions, non-Kalamazoo programs and foreign Consular Officials reserve the right to request additional materials and to alter deadlines. Please watch your mailbox for further notices and materials. Students participating in non-Kalamazoo programs are responsible for meeting all deadlines set by the administrators of their specific programs.)

* $300 non-refundable participation Deposit (checks should be made payable to "Kalamazoo College")
* Health History form
Due by Thursday, October 26, 2006

* Health Insurance Form and Proof of Insurance
* 10 Passport Photos:
1. Please write name, program and Student ID on back of each photo
2. Cover the writing with scotch tape to prevent smearing on other photos
3. Enclose photos in unsealed envelope with name, program and student ID written on envelope
* Two copies of Passport picture page
* Medical Evacuation Insurance Form
* Study Abroad Travel Form: Air Travel Return Date

 

* All materials, forms, and payments are due on the date specified. Failure to turn in items by the date they are due can jeopardize your participation in the program, and may lead to your withdrawal from the program.

Please check with Business Office to make sure your account is cleared. Your ticket will not be released unless your account is paid in full or appropriate arrangements have been made through the Kalamazoo College Business Office.
